These Terms and Conditions govern your use of applewhitedentalarts.com and any communications you receive from us, including text messages and email. By using this website, submitting a form, or opting in to receive messages from us, you agree to these Terms. If you do not agree, please do not use the site or opt in to our communications.

3. No Dental or Medical Advice
The content on this website is provided for general educational purposes only.
It is not dental or medical advice, and it is not a substitute for an examination, diagnosis, or treatment plan from a licensed dentist. Every patient is different. Treatment options, timelines, and costs described on this site are general and may not apply to you. Never delay seeking professional care because of something you read here.
4. No Doctor-Patient Relationship
Using this website, submitting a form, or exchanging messages with us does not create a doctor-patient relationship. That relationship begins only after you have been seen and accepted as a patient of the practice.
5. Appointment Requests
Requests submitted through this website or through our online scheduler are
requests, not confirmed appointments, until our office confirms them with you directly. We will do our best to accommodate your preferred time, but availability changes.
6. Text Message (SMS) Program
We offer two types of text messages, and you choose which ones you receive by checking the corresponding box on our website form. Neither box is checked by default.
- Marketing messages. Follow-ups about a consultation request you submitted, including a link to book a time, and occasional information about our services.
- Non-marketing messages. Appointment confirmations and reminders, and replies when we were unable to answer your call.
Consent is not a condition of purchase or of receiving care. You can decline both and still request an appointment, call us, or be treated at our practice.
Message frequency varies depending on your activity with us and your upcoming appointments.
Message and data rates may apply. These are charged by your mobile carrier, not by us. Check your plan for details.
To stop receiving messages, reply
STOP to any message from us. You will receive one confirmation that you have been unsubscribed, and no further messages after that. To rejoin, submit the form again or ask our office.

Carriers are not liable for delayed or undelivered messages.
You must be 18 years of age or older to opt in to our text message program.
We do not sell, rent, or share your mobile phone number or your SMS consent with third parties or affiliates for marketing purposes. SMS opt-in consent is never shared with anyone.
Do not use text messaging for emergencies or to send sensitive health information. Text messaging is not a secure channel. See Section 8.
7. Email Communications
If you provide your email address, we may send you appointment information and, where permitted, information about our services. Every marketing email includes an unsubscribe link. Unsubscribing from marketing email does not stop appointment-related email.
8. Not for Emergencies or Sensitive Information
If you are experiencing a dental or medical emergency, call 911 or go to your nearest emergency room. Do not use this website, our forms, text messages, or email to report an emergency. We do not monitor these channels around the clock.Please do not send detailed health information, insurance identifiers, or other sensitive personal information through website forms, text message, or unencrypted email. Call our office instead.
9. Eligibility and Your Responsibilities
You must be at least 18 years old to use this website, submit a form, or opt in to receive text messages or email from us. If you are under 18, please have a parent or legal guardian contact our office by phone.You agree to provide accurate information, and to only submit a phone number or email address that belongs to you or that you are authorized to use. You agree not to use this site to interfere with its operation, attempt unauthorized access, or submit unlawful or harmful content.

The content on this website, including text, images, photographs of our work, logos, and design, is owned by or licensed to Hedieh Pournik I PLLC and is protected by copyright and trademark law. You may view and print pages for your own personal, non-commercial use. Any other reproduction or distribution requires our written permission.
