Applewhite Dental Arts is a d/b/a of Hedieh Pournik I PLLC. We are serious about protecting your privacy. This Privacy Policy explains what information we collect, how we use it, and the choices you have.

"You" or "your" means you as a visitor to or user of this website. "We," "our," or "us" means Applewhite Dental Arts. "Our site" means https://www.applewhitedentalarts.com and any page on it.

1. What Information We Collect

Information you give us. When you submit a form on our site, we collect your first and last name, email address, mobile phone number, the service you indicated interest in, and any message you write.

Consent records. When you check a consent box, we record the exact wording of the box you checked, the date and time, and the IP address the submission came from. We keep this as proof of your consent.

Information collected automatically. When you browse our site we receive your IP address, browser, and operating system, which helps us understand how the site is used and improve it.

2. How We Use Your Information

We use what you give us to respond to your request, schedule and confirm appointments, and answer questions. Where you have given permission, we also use it to send you follow-up information about our services.

Email. With your permission we may send you emails about our services and practice news. Every marketing email includes an unsubscribe link.

Calls and text messages. With your consent we may call or text you to help set up an appointment and to ask how we can help. See Section 3.

3. Consent, Calls, and Text Messages

Our website form includes two separate checkboxes, neither of which is checked by default:

  • One grants consent to receive marketing text messages from us.
  • One grants consent to receive appointment reminders and other non-marketing text messages from us.

You may check either, both, or neither. Consent is not a condition of purchase or of receiving care. You can decline both and still request an appointment, call us, or be treated at our practice. We only send you a category of message if you checked that specific box.

Message frequency varies. Message and data rates may apply, charged by your mobile carrier and not by us. You must be 18 or older to opt in.

To withdraw consent at any time: reply STOP to any text message, click the unsubscribe link at the bottom of any marketing email, or call our office at (281) 469-8300 and ask us to remove you. Reply HELP to any text message for help.

4. Disclosure

We do not sell, rent, or otherwise disclose your personal information to any third party, including advertisers, strategic partners, or vendors, for their own marketing purposes.

This specifically includes your mobile phone number and your SMS or text messaging consent, which are never sold, rented, or shared with third parties or affiliates for any purpose.

We may disclose your personal information if we are required by law to do so, or if you violate our Terms & Conditions. If our practice is acquired or merged, your information may transfer to the new owners so they can continue to serve you.

5. Third-Party Services

We use third-party providers to operate this website, host our forms, and manage appointment scheduling and patient communication. These providers only access the information they need to perform their function on our behalf, and they are contractually restricted from using it for any other purpose.

Where those providers handle protected health information, we maintain the agreements required under HIPAA.

6. Security

To protect your personal information we take reasonable precautions and follow industry best practices to ensure it is not inappropriately lost, misused, accessed, disclosed, altered, or destroyed.

Please do not send detailed health information or other sensitive personal information through website forms, text message, or unencrypted email. These are not secure channels. Call our office instead.

We use cookies to create a more customized experience on our site. We also use analytics tools to measure where our website traffic comes from and how the site performs.

8. Your Rights and Choices

You may ask us to access, correct, amend, or delete the personal information we hold about you, unsubscribe from marketing email, stop text messages, or register a complaint. Contact us using the details in Section 11 and we will respond.
